Getting Started

Thank you for trusting us with your medical care! Our goal at Jordan MD Elite Primary Care is to meet all your health care needs as efficiently as possible. To help meet this goal, please take some time to read the following information:

Create your account for Elation Passport, our patient portal

When your membership was created, you received an email with a link to create your Passport account. Elation Passport is the very best way to communicate with Dr. Jordan and her staff. You can schedule appointments, request refills, send a message to Dr. Jordan, change your payment options and view all your contact information. If you did not receive the sign-up email, let us know and we can re-send you a link or provide guided help.​ You can also find the link on our homepage, but its a good idea to bookmark the page for easy access.

Download the Spruce App

Upon active membership, you are automatically assigned a Spruce account that is tied to your phone number. Spruce is our primary phone system. Anytime you call us, the call is routed through Spruce. While you can always call us or text us normally, using the Spruce app allows us both to send HIPAA secured messages to ensure your privacy is protected. Spruce is fine to get in touch; but as an established patient, we prefer you use Elation Passport as its more direct, more secure and ties in directly with your medical record for efficient handling of your requests. 

Understand our clinic hours and after-hours services

Our normal operating hours are 9am - 5pm. During those hours, we are here to answer the phone and see patients. For most of your needs, this is the best time to communicate with us. Outside of those hours, we don’t generally answer phone calls; we ask that you please send a message through the Patient Portal or Spruce, as this allows your concern to be addressed as soon as possible. Please allow 24 hours for a response to non-urgent issues. If you have an urgent issue after hours, please call our main number and select the option to reach Dr. Jordan.

Common Questions

How can I make an appointment with Dr. Jordan?

You can directly make an appointment using your Patient Portal account.  This is the fastest and easiest way to get the appointment you want.  You can also message us through Spruce or call our main phone number; but please keep in mind it might take a little longer to reach us this way.

How can I request medication refills?

You can send us a message through your Patient Portal account.  Dr. Jordan will evaluate the request and send it to the pharmacy for you.  Dr. Jordan may decide that she needs to have a visit with you before refilling the medication.  We will let you know if that is the case.

Do you offer after hours services?

Outside of our normal operating hours, we don’t generally answer the phone, but you can leave us messages.  If you have an urgent issue, there is an option to reach Dr. Jordan directly.  We do our best to work around your schedule.  As such, we can make appointments for you outside of our normal operating hours as necessary.

Should I go straight to an urgent care center or emergency room if I have a concern after hours?

No, this is not necessary (For serious emergencies, go to the ER, and then call us)

In non-emergency situations, please call us and select the option to reach Dr. Jordan directly.  Dr. Jordan can assess your situation and decide if Urgent Care/ER is appropriate.  Oftentimes, there is no need to seek emergency care and Dr. Jordan can help you manage your urgent medical concern.

You can send us a message through your Patient Portal account.  Dr. Jordan will evaluate the request and send it to the pharmacy for you.  Dr. Jordan may decide that she needs to have a visit with you before refilling the medication.  We will let you know if that is the case.
